What this form is for

This is the model withdrawal form set out in distance selling law. Use it to tell us you are withdrawing from the purchase within the 14 day period.

You do not have to use this model. Any unambiguous statement works, including an email in your own words to info@soulifyoga.com. The model exists so you can be sure nothing is missing.

How to send it

By email to info@soulifyoga.com, pasting the text above with the fields filled in. This is the fastest way and the one we recommend.

By post to Júlia Gabrielle Cristina Gomes de Abreu, Rua de Nossa Senhora de Lourdes 7, 2495-422 Fátima, Portugal. In that case the send date is what counts.

We confirm receipt by email within one working day. If you get no confirmation, chase us: your request is not lost because we failed to reply.

What happens next
